What is the acronym for these medical terms?

I'm going to school for a Medical Assistant and I have to find the acronym for the words. I know some of them but I'm having a hard time with these other ones. Can some one please help me? Here are the words. Premium, referral, Subscriber, and open panel HMO. Thank you so much in advance I really do appreciate the help..

1. SUB - Subscriber POP(can be used for premium only plan) RTT or AUTH for referral( referral to treatment or authorization) hope this helps.. not sure about open panel hmo sorry & good luck
