What does the term H/o stand for in medical terms?

I got a letter with that wording from my doctor and wanted to know what it means. Thanks!

2. H/o most of the time means "history of". Hx/o would also mean the same thing, just like "Hx". HO (with no slash) means Heterotopic ossification. Which is the abnormal development of bone in usually in soft tissue.
